The Xavier University of Louisiana College of Pharmacy, Center for Minority Health and Health Disparities, Research and Education will sponsor its Third Annual Health Disparities Conference.

The conference takes place April 19-21, 2009 in New Orleans, Louisiana and will focus on replicable multidisciplinary collaborative models and approaches from the clinical, research, and community arenas that integrate all levels of providers to improve health outcomes and eliminate health disparities.

The conference brings together clinicians (pharmacists, nurses, physician assistants, and other allied health professionals and physicians), social workers, health policy makers, health educators, researchers, and public and community health leaders around the theme “Building Partnerships to Eliminate Health Disparities: Effective Chronic Disease Management Models to Improve Health Outcomes.” 

This conference’s design history is one that allows participants to review and learn strategies on preventing and treating major chronic diseases and empowering communities’ clinical, scientific, academic, and public health agencies to improve health outcomes and eliminate health disparities through the use of successful collaborative models.  The main purpose of the conference has been to provide learning sessions and review and develop skills, techniques, and approaches, which influence outcomes through interdisciplinary comprehensive healthcare provider models.
